In general, procedures listed under Functional and Reconstructive procedures are covered as long as all of the requirements by the particular insurance plan are satisfied. Dr. Zemplenyi is a Preferred Provider for several plans including Regence Blue Shield, Premera Blue Cross, First Choice, Cigna, United HealthCare and Medicare. Some plans require a referral from the primary care physician.  Please check your Insurance Company PPO booklet for details. Feel free to call us at (425) 455-7225. Our office will obtain pre-authorization from your insurance company prior to any major procedure.

Health insurance companies never cover procedures listed under Cosmetic Plastic surgery. However, insurance may cover surgery for such problems as excessive droopiness of upper eyelids that interferes with vision, reconstructive nasal surgery for breathing and other problems caused by trauma. In addition, we frequently combine nasal cosmetic surgery with procedures to improve the function of the nose or sinuses . With proper pre-authorization, insurance companies pay for the functional component of the surgery. Dr. Zemplenyi and our staff are able to answer and counsel you about these issues.

During your consultation about Cosmetic Plastic procedures, you will receive a written quote for the cost of the procedures discussed. It is our office policy that fees for cosmetic surgery must be paid in full at least two weeks in advance of your surgery date if paid by a personal check. We accept certified checks the day of surgery. We accept VISA and MasterCard. Some of our patients also use finance companies such as CareCredit, Unicorn and CapitalOne.
